Delayed Enrollment form (This form needs to be submitted for ALL children that do not physically attend your program on the 1st day)

VPK Classroom Transfer Form (Please submit this original form if you are moving a child to a room other than what was put on their original certificate of eligibility or pilot paperwork.)
